At the heart of Freelandville Community Home is the nursing staff!

Our nurses are committed to providing each resident with the care they need and deserve. They are passionate about our residents’ well-being.

Nurses are given the optimal training environment and on-going CNA training programs, giving them the tools and education they need to serve our residents.

Freelandville Community Home strives to meet the needs of all our residents to the best of our ability. We achieve this with systems to ensure our facility is at its best. Our Medical Director is onsite to coordinate and progress medical services. Our Nursing Director oversees the healthcare administered at the facility.

We also have a psychologist and dietitian on staff to plan and provide appropriate care.  Each resident will receive an individualized care plan.

Transportation services are provided to residents as needed.

 

Dementia Care

Residents suffering from dementia will accommodated with simple routines and visual cues to help them flourish throughout each day.

Nurses will collaborate with residents’ families to make their stay at the facility as comfortable as possible.

Diabetes Management

 Our staff will combine all their expertise to create a plan to treat all aspects of diabetes. Nurses will help with monitoring blood glucose and insulin administration and tracking.

The residents will also be educated on nutrition’s effects on diabetes as well as given a menu plan. Exercise plans will be tailored to each resident’s capabilities.

    Restorative Nursing Program

     

    We care deeply about our residents’ quality of life which is what led us to include a restorative nursing program in the room and board fee.

    Restorative nursing program is care aimed to prevent any decline of daily living. This includes movement like walking, range of motion, maintaining strength, contractual management, positioning, and alignment.

    We also focus of rehabilitative dining, social dining, weight and height management, and bowel/bladder training.

    Dietary Program

    Our dietitians create a program for each resident based on nutritional needs and physician’s orders.

    We supply specialized meals, supplements, and regular nutritional assessments to keep residents on the right track.
